If it's his fault, then it's also his other friend's fault for not saying "let's stand over there in case parts fall off the building." And it's their parents' fault for not raising them to be more cautious. And etc/etc/etc.
I'm guessing the skull pretty much indicates his friend died as a result.
Looks like he managed to clutch his skull so probably not an immediate brain death. Maybe just enough of a hit that it started brain swelling which killed him before help could arrive or such.
Not sure who's calling for help but maybe there are others gathered who didn't show in the image.

Does it do the Grim Reaper routine like in this page, too? If yes, next time, (try to) casually reply "oh hey, how'd your bleaching go?" to give it pause.

Think the "It's your fault" is speaking about the shuttle crash. Not about that incident from his past, it's just a mental parallel.
